Eagle County 2017 Community Health Improvement Plan

Abstract: 

The Public Health Act requires local Public Health agencies to complete a community health assessment and create a local public health plan at least once every five years. The 2017 Eagle County Community Health Improvement Plan (CHIP) was developed in partnership with community organizations and community members in an effort to address health concerns in Eagle County.

The effort identified five health improvement areas:

  • healthy living
  • independently aging in place
  • connection to preventative care
  • mental health and substance abuse
  • and radon awareness and mitigation.

The 2017 CHIP provides an overview of Eagle County’s demographics and health indicators, and each of the five community priorities are provided with a road map on how to improve the top health issues from 2013 – 2017.
